After submitting your online registration form, you will receive an automated email from the CDC Vaccine Administration Management System (VAMS) within 48 hours. The email will come from no-reply@mail.vams.cdc.gov. Please check your spam and junk folders to make sure it didn’t get accidentally filtered incorrectly. You must complete the instructions in this email to schedule your vaccination appointment. Updated 1/21/2021 Can I come with my spouse or a family member? Yes, you can come with your spouse or family member, however they may only receive the vaccine if they have an appointment. When registering online, there is an option to indicate that you will be arriving at your appointment with another Phase 1B eligible member who would like to be vaccinated. That person will verify and complete registration paperwork onsite. During the online registration process, it is so important that this box is checked because this will help to ensure the appropriate amount of vaccine doses is available for that day at the vaccination site. Updated 1/21/2021 I am not a NH resident but I own property in NH, am I eligible to receive the vaccine in New Hampshire? No, you are not currently eligible to receive the vaccine in New Hampshire. Individuals will need to bring at least one of the documents below when they arrive to get their vaccine: Valid, unexpired NH Driver license or Non-Driver ID card. A payroll check, payroll document, or employment contract showing individual’s legal New Hampshire address, dated within the last 60 days. A government issued payment (ie. social security), showing individual’s legal New Hampshire address, dated within the last 60 days. After receiving your first vaccination dose, you will receive an automated email from no-reply@mail.vams.cdc.gov to schedule your second dose. Updated 1/21/2021 I got my first dose in another state; can I get the second dose here? Call 2-1-1 and select the option that you have questions about the COVID-19 vaccine. A representative from the NH COVID Coordinating Office will help you determine eligibility and schedule your second dose. Updated 1/21/2021 Will I get a card with vaccine information on it? Yes, you will receive a vaccination record card that documents your first dose. Bring this card with you to your second vaccination appointment and the card will be updated with your second dose information.
